 Location within Richmond, Emanuel, Jenkins, and JeffersonThis position is in East Central Health District but travels between Emanuel, Jefferson, Jenkins, and Richmond Counties.
 This position does not offer relocation assistance.  Must be within a reasonable commuting distance.   If relocating to the area, please specify on the application.

As a Perinatal PH Nurse within East Central Public Health District,you will join a dedicated group of public health professionals that help support the overall mission and vision of protecting lives of Georgian residents in a variety of ways.
Starting Salary $60,000

Job Responsibilities

Position will provide home health visits in Richmond, Emanuel, Jenkins, and Jefferson County.
Travel is reimbursed at the State of Georgia mileage rate.

Under broad supervision, provides professional nursing care in accordance with nurse protocols, policies and/or procedures.
Focuses on the prevention of illness, injury, or disability, the promotion of health and maintenance of the health of populations. Cares for populations with special needs during disasters/emergencies and biological and chemical protocols.
Identify high risk populations and tailor interventions based on the needs of the population and resources available.
Provide home visiting services to identify, assess and provide care coordination services to eligible perinatal patients and qualifying infants.
Coordinates program services with other service providers and community organizations to promote optimal implementation.
Screen and refer clients to other community service providers.
Monitoring per program guidelines, educational instruction, materials to promote wellness, safety, and pregnancy and postpartum complications.
Monitor for infant complications to include feeding, weight, head circumference, and developmental screening.
Develop a plan of care for individuals and families, modify treatment plans as indicated by client’s responses and conditions.
Provide referral/linkage when medical and environmental risks are identified to appropriate services.
Promote program through community engagement and outreach events. Develop and sustain community partnerships.

Minimum Qualifications

Bachelor’s degree in nursing from an accredited college or university AND current Georgia license to practice as a Registered Professional Nurse AND CPR certification,
OR associate degree in nursing from an accredited, college or university AND One year of nursing experience AND current Georgia Registered Professional Nurse license AND CPR certification.
Must have the ability to work independently, be self-motivated and have exceptional time-management, interpersonal and organizational skills. Maintain knowledge of and seek out new community resources.
Have reliable transportation.

Preferred Qualifications:
Perinatal and/or infant experience
Bilingual, computer skills, and outreach service experience
